Old Approval, Thin Head-to-Head Data
In a 1993 study (Delemarre-van de Waal, n=38) of men with hypogonadotropic hypogonadism, pulsatile GnRH administration restored testicular growth and sperm production. Gonadorelin's use for fertility and diagnostic purposes has decades-old FDA approval. However, head-to-head trial evidence comparing gonadorelin against hCG — the more established TRT-adjunct — for preserving fertility during testosterone therapy is mixed and limited.
Regulatory and Legal Status
Gonadorelin is prescription-only and carries older FDA approval for fertility and diagnostic use. Its use alongside TRT to preserve testicular function is off-label but a legitimate prescribed use through a licensed provider — it is not a controlled substance.
