How this is graded
Every compound below carries a REGEN research grade, from S (regulatory approval behind the use it is bought for) down to F (no clinical evidence). Ordering follows that grade rather than search volume, so the compound at the top is the one with the most evidence, not the one most often sold. Doses are those reported in published protocols and are not recommendations.
1. GHK-Cu
GHK-Cu is a naturally occurring copper-binding tripeptide whose plasma levels decline with age. It is best known for stimulating collagen and elastin production, improving skin firmness and wound healing, and supporting hair follicle health. It also has antioxidant and anti-inflammatory properties and has been studied for gene-expression effects associated with tissue remodeling. Available both as an injectable for systemic effects and topically for skin and hair. Often part of longevity and aesthetics protocols rather than performance stacks.

2. NAD+
NAD+ (Nicotinamide Adenine Dinucleotide) is a coenzyme found in every living cell, central to mitochondrial energy production and DNA-repair enzyme activity. Cellular NAD+ levels decline with age, which has driven a wave of consumer interest in supplementing it directly via IV infusion, injection, or oral precursors. Unlike a conventional drug, NAD+ doesn't behave with a simple circulating half-life — it's rapidly taken up and used intracellularly. It's sold over-the-counter as an oral dietary supplement, and administered by IV or injection at wellness clinics, but it is not an FDA-approved drug for any indication, and anti-aging claims made about it are not FDA-evaluated.
3. Glutathione
Glutathione is a tripeptide (glutamate-cysteine-glycine) produced naturally in the liver and considered the body's primary intracellular antioxidant, neutralizing free radicals and supporting detoxification pathways. Though technically a tripeptide, it's marketed and used almost exclusively as a wellness supplement rather than a research peptide — sold as an oral/liposomal capsule and as an IV or intramuscular injectable at wellness clinics, often alongside NAD+ on the same infusion menu. It's regulated as a dietary/food ingredient rather than an approved drug, and injectable glutathione marketed specifically for skin lightening has drawn FDA scrutiny.
4. Thymosin Alpha-1
Thymosin Alpha-1 is a naturally occurring 28-amino-acid peptide fragment of prothymosin alpha, produced by the thymus gland and involved in T-cell maturation and innate immune signaling (notably through TLR9). Under the brand name Zadaxin, it's an approved drug in more than 35 countries — including Italy and China — for chronic hepatitis B and C and as a general immune-modulating agent. It has never been FDA-approved in the United States; as of a February 2026 reclassification, it holds FDA 503A Category 2 status, meaning US patients can access it only by prescription through licensed compounding pharmacies.
5. Epithalon
Epithalon (also spelled Epitalon) is a synthetic tetrapeptide with the sequence Ala-Glu-Asp-Gly (AEDG), designed as an analog of epithalamin, a naturally occurring pineal gland peptide. It has been studied — primarily by Russian researchers, notably Vladimir Khavinson's group — in small and preclinical studies for telomerase activation, pineal gland/melatonin regulation, and circadian rhythm effects. It's sold exclusively as a research compound; no FDA approval or large-scale human trial exists.
6. SS-31 (Elamipretide)
SS-31 (elamipretide) is a small, mitochondria-targeting tetrapeptide that concentrates in the inner mitochondrial membrane and stabilizes cardiolipin, a lipid essential to mitochondrial energy production. It received FDA approval in September 2025 for Barth syndrome, a rare inherited mitochondrial muscle disease, after an extension study showed meaningful improvement in a 6-minute walk test. That approval is genuinely narrow, though: a larger 218-person trial in a different mitochondrial-muscle condition (MMPOWER-3) showed no improvement in walking distance or fatigue, and separate heart-failure and dry-AMD trials both missed their primary endpoints. It's grouped with longevity research because of its mitochondrial mechanism, but that broader case rests mostly on animal data plus this one rare-disease approval.
- Muscle Function (mitochondrial disease) (grade B) — FDA-approved Sept 2025 for Barth syndrome (TAZPOWER extension); MMPOWER-3 (n=218) in a different mitochondrial-muscle condition showed no gain in walking or fatigue.
- Longevity / Mitochondrial Health (grade C) — Heart-failure and dry-AMD trials both missed primary endpoints; general longevity case rests on animal data plus the narrow rare-disease approval.
